Information required:	Personal information. Account information. Payment terms. Copy of a personal cheque from your financial institution clearly marked VOID.
Tip:	Print all the information in uppercase letters. Be sure to complete all sections of the form. Check off your choice of payment option. See the example below to complete the "Transit Number" and "Savings Chequing Account Number" sections.
