Claims

All Credit Balance Insurance - Business claims must be submitted to Desjardins Financial Security (DFS):

• by phone at 1 866 838-7532
• by writing to:
           Desjardins Financial Security
           200, avenue des Commandeurs
           Lévis (Québec) G6V 6R2

Desjardins Financial Security may ask you to provide, as quickly as possible, additional information required to assess your claim.

In case of disability

You may submit a claim once you have been totally disabled for at least:

• 31 consecutive days (benefits are payable as of the 1st day)

Termination of disability payments

To avoid having to pay back overpaid benefits, you must contact Desjardins Financial Security at 1-866-838-7532 if you:

• go back to work;
• go back to school or get job training;
• are no longer totally disabled.

In case of death

It is important to include the insured's death certificate with your claim.

In case of critical illness

You may submit a claim as of the 31st day following the initial diagnosis of a critical illness¹.

1. Critical illness means cancer, heart attack, stroke or terminal illness diagnosed by a physician.
