How to fill out forms and applications

The instructions page

Each application form is preceded by an instructions page that tells you everything you need to know before filling out the form. Instruction pages include:

1. an introduction, identified by a thumbtack icon, giving you the most important information
2. a table telling you what information you will need to gather before you fill out the form, the average time required to fill out the form and the application processing time
3. a link to the secure application form
4. a note on privacy and security
5. phone numbers you may call if you need assistance while filling out the application
